Important
Reminders:
Payment must be enclosed with your registration.
Acceptable forms of payment include check
(drawn on U.S. bank), Visa, MasterCard,
or government voucher/purchase order.
Government vouchers/purchase orders are
subject to an additional $10 processing
fee.
All paperwork must be attached to the
registration form.
Please retain a copy of your registration
for your records. If your registration
is received by September 15 you
will receive confirmation. If you have
not sent in your registration by September
15 please hold it and register on site.
Special Note: Admittance to
all workshop sessions will be on a first-come,
first-served basis at the beginning of
each conference session. Room monitors
are available to assist you in locating
available seating. Standing will not be
permitted, so please do not overcrowd
sessions. We thank you for your cooperation.
Hotels
and Reservations
Full-Time
Students
To qualify for the full-time student rate,
you must provide a current tuition payment
receipt, a letter from a professor, or
copy of a dated student ID card that verifies
full-time study (12 credits of undergraduate
or 9 credits of graduate work).
If verification is not enclosed with
your registration, you will be charged
the appropriate full rate.
Cancellations
and Refunds
Requests for refunds must be made in
writing and must be postmarked by September
15 for a full refund, less a
$50 administrative service charge. The
cancellation will be acknowledged, but
refunds will not be mailed until after
the conference.
Cancellations postmarked between September
15 and September 29 must include documentation
of a medical emergency. Those unable to
attend because of a medical emergency
will be issued a letter of credit for
50 percent of this year’s fee that
can be applied towards ACR’s 2005
conference registration fees.
Refund requests postmarked after September
29 cannot be honored under any circumstances.

Substitutions
Attendee substitutions are permitted
at any time upon written notice of the
original registrant. The appropriate member/non-member
rate will be applied to the substitute
attendee.
Prospective
Members:
 ACR
members receive an $80 discount on conference
registration fees, in
addition to all the other membership benefits.
If you are not already a member, you can
qualify for the member conference registration
rates by applying for membership at the
time you register.
