Important Registration Guidelines:

  • Payment must be enclosed with your registration. Acceptable forms of payment include check (drawn on U.S. bank), Visa, MasterCard, American Express, or government voucher/purchase order. No other forms of payment will be accepted. (Government vouchers/purchase orders are subject to an additional $10 processing fee. For those paying with a government voucher/purchase order, all paper work must be attached to the registration form.)

  • All ACR members must indicate their ACR Member Number on the registration form. We need to know your member number so we can offer you the member discount. If you don't know your member number, you can find it by logging in to the ACR Member Center.

  • Please retain a copy of your registration for your records.
    Registration confirmation will be sent by e-mail within two weeks of receipt of your registration. If your registration is received by October 1 you will receive confirmation. Registrations received after October 1 will be processed on-site. If you have not sent in your registration by October 1, please hold on to it and register on-site. Photocopies of the registration form are acceptable.

  • To qualify for the full-time student rate, you must provide a current tuition payment receipt, a letter from a professor, or copy of a dated student ID card that verifies full-time study (12 credits undergraduate or 9 credits graduate). If verification is not enclosed with your registration, you will be charged the appropriate full rate.

  • Expenses for continuing professional education (including registration fees, lodging, and meals) undertaken to maintain and improve professional skills are generally tax deductible. Please consult with your tax advisor.

  • Requests for refunds must be made in writing and must be postmarked by September 15, 2003 for a full refund, less a $50 administrative service charge. The cancellation will be acknowledged, but refunds will not be mailed until after November 15, 2003. Cancellations postmarked between September 16 and October 15 must include documentation of a medical emergency. Those unable to attend because of a medical emergency will be issued a letter of credit for 50% of this year's fee that can be applied towards ACR's 2004 conference registration fees. Refund requests postmarked after October 15 cannot be honored under any circumstances.

  • Attendee substitutions are permitted at any time upon written notice of the original registrant.
